Twelve Minutes, Township or Borough

Freehold is two places with one name — Freehold Township wrapped around Freehold Borough — and we see patients from both. From the Freehold Raceway Mall it is about twelve minutes to our Marlboro office: Route 9 north to West Main Street, then Marlboro Road. From the borough centre and the county offices it is a similar trip.

As the county seat and the area's retail centre, Freehold has a working population that spends a great deal of the day upright and moving — and standing all day is its own kind of load on a spine, distinct from sitting all day and treated differently.

What We See Most Often From Freehold

  • Low back pain from prolonged standing. Retail, hospitality, healthcare and trades all put people on hard floors for eight or ten hours. The pattern is a lower back that is fine in the morning, aching by mid-afternoon and stiff by the end of a shift, often with the pain easing as soon as you sit — which is the opposite of the desk-worker pattern and points at different structures. Back pain relief begins with establishing which.
  • Sciatica. Where sustained standing or repeated lifting puts a disc under pressure, the resulting nerve irritation sends pain into the buttock and down the leg.   • Feet, hips and the chain above them. Standing problems rarely stay local. What begins as foot or hip discomfort changes how you stand, which changes how the pelvis and lumbar spine are loaded, which produces back pain that appears unrelated to the original complaint.
  • Stubborn inflammation. Soft-tissue injuries that are never rested — because the shift still has to be worked — settle slowly. Class IV laser therapy is often part of the plan for these.

We also see a steady number of pregnancy-related complaints from Freehold. If you are on your feet all day, the examination will also cover what is realistic. Care that requires you to stop working is not care most people can take, and it is usually not necessary — but the plan has to be built around the shift pattern rather than in spite of it.

What care looks like after that

Adjustment restores motion where a joint has lost it. Where a disc is under pressure, spinal decompression reduces the load on the irritated structures. Laser therapy helps where inflammation is the limiting factor. When to Stop Waiting It Out

  • Back pain that arrives reliably by the end of every shift
  • Pain travelling into the buttock, thigh or below the knee
  • Numbness, tingling or weakness — worth prompt attention
  • Pain that has started dictating which shifts you can take
  • A complaint that eases on days off and returns within one working day

That last pattern is the clearest signal that the cause is mechanical and ongoing rather than an injury that is healing.

Hard Floors, and What Actually Helps

Standing all day is not one problem, it is three, and they respond to different things.

The first is the surface. Concrete and tile return every bit of force back up through the foot, ankle, knee, hip and lower back. An anti-fatigue mat at a till or a workstation is not a comfort item — it changes the load meaningfully, and it is usually the single cheapest intervention available. Where a mat is not possible, footwear is doing that job instead, which is why shoes that have been worn flat are a genuine cause of back pain rather than an excuse for one.

The second is stillness. Standing still is harder on a lower back than walking is, because walking alternates the load between sides and standing does not. This is why retail staff on a slow shift often feel worse than on a busy one. Shifting weight deliberately, or using a low rail or box to rest one foot on and swapping sides periodically, breaks that up.

The third is what happens after. A back that has been extended and compressed for ten hours does not want to go straight to a sofa in the same posture. A few minutes of moving in the opposite direction — gentle forward bending, walking — does more than an hour of sitting still.

None of these replace treatment where a joint has already lost motion or a disc is involved. They do decide whether the treatment holds, and they are worth starting before the first appointment rather than after it.

I stand all day at work. Is that different from sitting all day?

Mechanically, yes. Prolonged standing and prolonged sitting load different structures and typically produce opposite patterns — standing pain tends to ease when you sit, sitting pain tends to ease when you move. The examination establishes which one you have.

Will I have to take time off work?

Usually not. Care that requires you to stop working is care most people cannot take, so the plan is built around your shift pattern. Occasionally a specific movement needs modifying for a period, and we will say so plainly if that is the case.
